Definitions provided by the Medical Library Association in the United States of common medical terms used in the media.

ISMP's List of Error-Prone Abbreviations, Symbols and Dose Designations

A list of abbreviations and other symbols that the Institute for Safe Medication Practices (ISMP) recommends should never be used. These have been reported to the ISMP via their Medication Error Reporting Programme as being often misinterpreted, causing medication errors. This is for information only; check with your respective organisation for any in-house rules on abbreviation use that may apply.

Official "Do Not Use" List

Abbreviations and symbols it is advised not to use on handwritten orders and other medication documentation. The "Do Not Use List" is produced by the Joint Commission, a body that provides accreditation services for healthcare organisations and programmes in the US.
