Pre-surgery information checklist

The pre-surgery information checklist is a simple way for patients to check that they have received important information from the surgeon performing the procedure, and from the hospital, before they consent to cosmetic surgery.

It provides a mechanism for services to audit the delivery of important pre-operative information to the patient. The audit report and any subsequent improvement plan can form part of intelligence available for Care Quality Commission (CQC) inspections.

If you would like to order hard copies of these forms, the RCS can arrange for these to be printed and delivered to you. Request a quote
